Diabetic patients will be routinely recalled to the Diabetic clinics which are run by our practice nurses and Healthcare Assistants.
You will receive a blood test form and questionnaire. One of our Diabetic Team will contact you once we have received your completed form and blood results.
New diabetic patients will be offered a referral to a structured education programme called DESMOND. The aim is to help you manage your diabetes and maintain good health in the long term.
The sessions are run locally, either face-to-face or online.
