If you require vaccination for travel then please complete a TR01 form and return it to the surgery.
This will be assessed by our Nursing team who will advise based on the latest guidance available and make an appointment if required.
Please ensure that you do this, leaving as much time as possible before your travel date, as many vaccines require a course of injections. If you do not leave sufficient time then you will not be fully immunised and we strongly advise that you inform your travel insurer of this before you travel.
