Registration

In order to ensure we continue to give a quality service it may be necessary to manage the number of patients on our list. The criteria for accepting patients is available on request and MAY BE subject to change.

You can also collect a copy of the registration form from Reception.

If your details are found on the NHS records system we will register you as a patient.  If we are unable to find your details we will contact you to request to see various forms of personal ID.
